Office Policies
Scheduling appointments We do not offer walk-in appointments. We ask that you arrive at the office a few minutes before your scheduled appointment time so that patients can be checked in by the appointment time. Patients who arrive more than 15 minutes late may need to be rescheduled. To schedule an appointment please use the contact information on the Burlingame or San Mateo office page. Prescription RefillsPlease ask your pharmacy to send a request to our office for all refills. We will fax back refill requests to your pharmacy within 24 hours on weekdays. Weekend and Holiday refill requests will be completed on the next business day. Please allow 24 hours for your refill request to be processed. School/Childcare/Camp/Sport Forms & LettersEffective March 15, 2010, Peninsula Pediatric Medical Group will begin charging $10 to complete all school/childcare/camp & sport forms. We regret this decision but the volume and complexity of many forms has added considerable staff & physician time to complete & sign the forms. Physicians must review patient charts to verify that all relevant information is included on the forms. Please complete the parent’s section of the form before submitting forms to our office. Forms are usually completed within 3 business days. If your primary pediatrician is on vacation, the form completion may take up to 5 business days to complete. Payment will be collected when the forms are brought to the office. We are unable to bill parents or your insurance for this charge. Forms faxed or mailed in will be completed after we receive payment by check or credit card. Letters requested by parents for documentation of a child’s medical condition because the child is unable to participate in a camp, cruise, vacation or travel will be charged $25. Requests for letters appealing insurance denial or for appeals are also charged depending on preparation time. There is no charge for school PE excuses, return to school notes, mail-in prescriptions or WIC forms. Please speak with your child’s pediatrician if you have questions. Thank you. Cancellation PolicyIf you must cancel your appointment, please call us as soon as possible. Failure to call to cancel an appointment may result in a charge to you, which is not covered by insurance companies. Red Flag RuleThe Red Flag Rule is a new regulation that will be instituted by the Federal Trade Commission to help fight identity theft. Under these guidelines, Peninsula Pediatric Medical Group is required to ask for photo identification from all parents/guardians bringing any minor in for an appointment. Patients 16 and up will be asked for their own identification when applicable. We apologize for any inconvenience this may cause.
